Patient PathwaysSM, a provider-neutral discharge-planning and placement program, can enhance a hospital’s case management team capacity while providing specialized support by:
- Coordinating dialysis center placements for new and medically and financially complex patients requiring outpatient dialysis
- Increasing coordination across the care team to facilitate a timely dialysis chair placement
- Educating patients about dialysis, including what to expect and their options
- Facilitating a smooth transition for patients returning to their dialysis centers after hospitalization
- Enhancing analytics and transparency into key metrics
- Focusing on patient satisfaction
Patient Pathways can complement the case management staff with support from a trained placement expert to educate and place your most complex and time consuming dialysis patient discharges. This additional support can save an average of six hours for each discharge that includes a dialysis placement, which can reduce the burden on staff. This can also allow for better optimization of the existing case management team and help streamline workflows for the complex ESRD population. We focus exclusively on renal patients, serving as a single point of contact for the case management staff and nephrologists. Our teammates will coordinate across the local outpatient renal community, while using our proprietary workflows and best practices to help navigate and overcome common placement-related barriers to discharge. As part of a national team serving more than 250 hospital partners, our patient relations liaisons are committed to efficiently placing patients at a center of their choice while educating them with compassion and understanding.

What our hospital partners are saying about Patient Pathways
“We used to struggle trying to find an available slot at an outpatient dialysis center. It took the case manager away from other patients. Those problems have all but disappeared.”
“We have complex discharges. Our patients are frequently undocumented, have behavioral issues causing placement issues [or] have transportation issues. [Pathways] has proven to be exceptional and works well in tandem with social work, our clinics and the chronic dialysis community.”
“It has taken away more hours than expected from the case managers’ workloads having [Pathways] coordinate the dialysis needs of our patients, especially for new dialysis patients. The patients get better education and support…than in the past when case managers would do a poor job of facilitating dialysis placement.”
“…[Pathways] has taken a tremendous amount of work off the case managers so they can focus on other issues. [Pathways] has easily fit into our department and works seamlessly with case management.”
